Whether you are visiting the hospital for the first time or follow up post- surgery, it is advisable to keep a few things in mind for your convenience and to avoid unnecessary delay.
In case of first visit to the hospital, the patient is advised to arrive 15minutes before the scheduled appointment time - so that he/she can be comfortable and have enough time to fill the forms at the hospital. You can download the patient registration form online.
In case the patient needs to cancel or reschedule an appointment, it is better to inform us well in advance.
Documents patient should bring along at the time of appointment:
Medical Records
Patient must carry a record of his/her previous illnesses and medical treatment from previous physicians. In case the case is being referred by a physician, the patient can request him/her to send a note to the hospital directly as well.
X-Rays and/or Scans
If the patient has recently gone through an x-ray or scan, he/she should bring the report along.
Medications
Patient should bring a list of all medications he/she is taking currently, or has taken recently. The strength and dosage of each medication should be noted in the list.
