To make pre-registration simpler and less time consuming, we have online patient registration forms. In case you so desire, you can fill the forms at the time of your visit to the Beams hospital. You can also take a printout of the form, fill it and carry it to the hospital with you.
If you plan to fill the forms in the hospital, we request you to reach the hospital 10 minutes prior to your appointment. Our Patient Relationship Manager will readily assist you in filling the form.
The patient would be requested to get admitted in the hospital either on the day of the surgery or one-day prior depending on the surgery. The patient must strictly follow the instructions about diet before getting admitted to the hospital. At admission a few forms would need to be signed by the person getting admitted and the relative.
